INDEPENDENT LIVING SKILLS TRAINER I - CNA

Reporting Responsibilities: Reports to the ILP Manager and Executive Director.

Position Status: Full time, non-exempt.

Basic Function: Provides intensive one-on-one skills training as well as facilitates groups and classes to participants involved in CPWD’s Independent Living Program. Performs, as needed, the duties of a Certified Nursing Assistant during Adult Day Program hours.

Duties and Responsibilities:

  • Provide intensive one-on-one independent living skills training to participants of the Independent Living Program. (IL skills training includes, but is not limited to bus training, menu planning, grocery shopping, laundry, cleaning, cooking, money management, etc.).
  • Must be licensed as Certified Nursing Assistant or willing to obtain licensure.
  • While at the Day Program, assist consumers and perform all functions that are within the CNA scope of practice.
  • Accompany consumer to clinic, physician’s office or on other trips as needed for treatment in accordance with the Plan of Care. 
  • Conduct Independent Living Assessments to determine real abilities and identify areas that require intensive one-on-one.
  • Assist consumers to identify goals and develop an independent living plan.
  • Review progress on IL plans and intervene with consumers and other staff to effect changes in progress.
  • Provide information and referral services to the consumer.
  • Plan and facilitate classes/groups for participants of CPWD’s Adult Day Program.
  • Maintain consumer files and all records required by CPWD.
  • Maintain attendance records.
  • Complete and maintain monthly billing forms.
  • Drive CPWD vans for program activities, etc.
  • Must possess knowledge of community resources in order to effectively advocate for consumer needs.
  • Work as a team member to assure that consumers receive quality services.
  • Attend and participate in consumer care conferences, CPWD staff meetings, program meetings & staff training activities.
  • Work effectively with various agencies to promote consumer needs.
  • Maintain 12 CEUs per calendar year.

Qualifications:

  • AA in Human Services or related field and/or a minimum of two years experience working with individuals with disabilities. 
  • Completion of Home Health Aide training program approved by the State of Colorado Board of Nursing (must obtain Colorado CNA license within 4 months of hire date).
  • Two years experience working with people with disabilities, preferably in an independent living center setting.
  • Demonstrated ability to be an effective communicator orally and in writing.
  • Knowledge of Independent Living Philosophy.
  • Demonstrated ability to work effectively with professionals and with the public.
  • Knowledge of Microsoft Office products including Word and Access.
  • Demonstrated ability to utilize e-mail and the Internet.


Desirable Qualifications:

  • Bi-lingual English/Spanish or English/ASL.
  • Personal experience with a disability.

Skills/ Special Requirements:

  • Knowledge of principles and procedures of personal care and safety practices applicable to CNAs.
  • Ability to lift 50 lbs
  • Must have reliable transportation, valid Colorado driver’s license and insurance
  • Successful completion of skills competency evaluation
  • Ability to establish and maintain effective working relationships with consumers, families, staff members, and the general public.
