Job Description Nursing Supervisor
Reporting Relationship: Home Health Administrator
Position Status: Full time, exempt.
Basic Function:
Provides skilled nursing care in accordance with the consumer’s plan of treatment including comprehensive health and psychosocial evaluation, monitoring of the consumer’s condition, health promotion and prevention, coordination of services, teaching and training activities and provision of direct nursing care. Exercises supervision of assigned personnel including licensed registered nurses, licensed practical nurses, home health aides, therapists and attendants.
Duties and Responsibilities:
- Performs initial evaluation of new consumer to develop plan of care in coordination with consumer, family, physician and health care team.
- Provides those services requiring substantial and specialized nursing skills
- Initiates appropriate preventative and rehabilitative care per MD orders
- Evaluates the effectiveness and outcomes of care, and initiates necessary changes to plan of care in consultation with MD
- Responsible for ensuring all OASIS, 485’s, 487’s, re-certifications and discharges are done and paperwork into chart in a timely manner
- Responsible for ensuring all OASIS are transmitted within specified time frames and that appropriate reports are generated
- Prepares clinical and progress notes, coordinates services, informs the physician and other personnel of changes in the consumer’s condition and needs
- Provides necessary training to staff members, family and consumer’s as needed
- Makes appropriate referrals to community and agency services
- Understands and complies with all appropriate HIPAA regulations. Assures and oversees strict client health care record privacy and confidentiality
- Ensures all MD orders are signed and in charts in timely manner
- Provides skilled nursing care under the orders of the consumer’s primary care physician and in accordance with accepted standards of practice
- Assists Home Health Administrator and PAS Manager in yearly performance evaluations and skills competency evaluations of staff members
- Acts as a liaison between agency MD’s, inpatient and outpatient facilities in the community
- Understands and ensures all nursing personnel are trained and comply with Medicare, Medicaid regulations, in addition to maintaining practice within their appropriate scope of practice
- Acts as Administrator when Home Health Administrator is absent
- Participates in all staff meetings, CNA meetings, and other committees as requested
- Assists with Clinical Record Reviews on a quarterly basis per Home Health Administrator’s request.
- Ensures all orders and plans of care are followed by staff members
- Assigns and schedules nursing visits to appropriate personnel.
Preferred Education and Experience:
- A baccalaureate or associate degree in nursing from a school accredited by the National League of Nursing.
- Minimum of 2 years home health experience.
- Management experience preferred.
Skills / Qualifications:
- Must have current license to practice as a Registered Nurse in the State of Colorado. It is the employee’s responsibility to inform CPWD within 24 hours of licensure expiration, revocation or investigation
- Ability to organize details, manage staff and projects, and function independently
- Possess effective leadership qualities
- Understanding and knowledge of Independent Living Philosophy
- Knowledge of the development of plans of care, and the ability to empower consumers to take charge of their care
- Ability to communicate effectively both orally and in writing
- Valid, current Colorado driver’s license and insurance
- Reliable transportation
- Availability to work week ends / on call on a rotating basis.
