Community Transition Services (CTS)

The Community Transition Services (CTS) is a state program to assist people with significant disabilities to transition from institutional settings into their own homes. The intention of the project is to help individuals with disabilities, who are living in skilled nursing facilities and are on Medicaid, transition from the nursing facility back into a community integrated setting.

Each independent living center in the state of Colorado has a staff member who is designated to be the Navigator for the project. This person, along with the resident, creates a comprehensive plan for transitioning to community living. The plan includes housing, budgeting, community support systems and home-health services.
